Healthy Communities

A community health needs assessment provides the foundation for improving and promoting the health of the community. The assessment identifies and describes factors that affect the health of a population, and factors that determine the availability of resources within the community to adequately address health concerns.

Bismarck-Burleigh Public Health, in collaboration with Custer Health, CHI St. Alexius Health and Sanford Health developed a multi-faceted assessment process designed to identify the health concerns of residents in the Bismarck-Mandan-Burleigh-Morton area.

The community health improvement plan summarizes the assessment process and identifies community health priorities. Key concerns will be addressed through implementation strategies in the next three years.

  • Healthy Living (Chronic Health Issues/Access to Healthy Food)
  • Access to Affordable Health Care
  • Affordable Housing
  • Access to Health Care Providers
  • Public Transportation
  • Long-Term Care
